Tattoo Release Form 

Please complete ALL questions below within 24 hours of your tattoo appointment at Tattoo Art Chicago.

We have added new regulations to our already strict studio code of cleanliness and sterility. One of these regulations is to have clients fill out our Tattoo Release Form online BEFORE you arrive at the studio. This minimizes time in the studio and creates a touch less check-in process.

Tattoo Art Chicago will need you to fill out the release form before we can proceed with vour appointment.

Please read through this Online Client Release Form carefully, fill it out thoroughly and let us know if you have any questions or concerns before your tattoo consultation or appointment.

AS ALWAYS, YOU MUST BRING A VALID PHOTO ID TO YOUR APPOINTMENT AND BE AT LEAST 18 YEARS OF AGE.
